Hazelwood Mental Health was founded on legacy—and is now growing on purpose.
The name Hazelwood honors two deeply influential figures: Dr. Palpu Hazel, a beloved psychiatrist who served the Valley for over 40 years with clinical excellence and heartfelt care; and Hazel Dixon, the strong and nurturing matriarch of Chyllia Dixon’s family. As Dr. Hazel retired, a meaningful synchronicity brought Chyllia into the practice. She supported his patients through the transition, ensuring they continued to feel seen, supported, and safe—hallmarks of the Hazelwood experience.
Now leading the practice, Chyllia Dixon, PMHNP-BC, a board-certified Psychiatric Nurse Practitioner, author, and recognized expert in psychiatric nursing, is shaping Hazelwood into one of Arizona’s premier boutique mental health clinics. With her accomplished academic background, leadership in evidence-based care, and passion for whole-person healing, Chyllia is building a provider team that reflects the best of both science and soul.
Hand-selecting each provider and support staff member, Hazelwood prioritizes clinical excellence, compassion, and consistency in every patient interaction. With growing referrals from partner practices across the Valley, Hazelwood is known for improving outcomes across a wide spectrum of psychiatric conditions—from depression and anxiety to complex trauma and developmental needs.
The practice is expanding—welcoming a third Psychiatric NP, enhancing its extended services partner network, and preparing to launch community wellness events including a series entitled "Meditation and Movement" set to open registration late-Summer of 2025. This next chapter of Hazelwood continues the legacy of its namesakes—offering expert care rooted in compassion, and a healing environment where patients of all ages can thrive.
